Understanding Your Health Data

Each time you visit a Trinity Dental Office or provider, a record of your visit is made. This record contains your Symptoms, examinations, test results, Diagnoses, Treatment, and a plan for future care. This information can often be referred to as your health or medical record. Also, to help plan your Care/Treatment, these records help communication among the dental professionals who contribute to your care.

Treatment – Information gathered by our dental team will be recorded in your chart and used to determine the best course of care for you. We may also share relevant reports with other providers involved in your treatment.

Payment – We may send a bill to you or a third-party payer that includes information identifying you, along with your diagnosis, procedures performed, and supplies used.

Health Operations – We may use your information for risk management, quality improvement, and business operations. When we work with outside business associates such as labs or billing services, we require them to safeguard your information appropriately.

Public Health and Legal Requirements – We may disclose your information as required by law to public health authorities, law enforcement, workers’ compensation programs, the FDA, or other legal authorities.
